How The Program Works

Clear responsibilities from eligibility to delivery.

  1. 1

    SCN Screening & Eligibility

Responsible: Social Care Network. The Social Care Network screens the member and determines eligibility.

  2. 2

    Service Authorization

Responsible: Social Care Network. The authorized service is referred through the appropriate network workflow.

  3. 3

    Clinical Nutrition Workflow

Responsible: Registered Dietitian. Where required, a member-specific Diet Order is issued under Registered Dietitian Nutritionist supervision.

  4. 4

    GT Food Service

Responsible: GT Food Group. GT prepares and delivers the authorized meal or food-box service.

  5. 5

    Documentation & Ongoing Review

Responsible: GT Food Group. Service documentation supports renewal, modification, or completion of the authorized service.

GT does not screen, enroll, or determine HRSN eligibility. Eligibility remains with the member’s Social Care Network.
